Burroughs has a secure electronic health records system through which we maintain current and comprehensive information on your children. This system allows us to provide the best care for our students and provides a safe school environment for all.If your children have any additional health needs at school or if you have any questions about the medical forms, please contact our school nurse, Casie Tomlinson, RN, BSN, MA.
DEADLINE
All health forms for all grades are due by Monday, August 12, 2024. Students with incomplete health information CANNOT pick up their class schedule or participate in fall sports practices until their health records are updated.

Double check all information for accuracy and completion, and remember to include signatures where needed. We recommend making a copy for your own records in case you need to reference what you have submitted.
Concussion Information
As part of our concussion management program, all new students will be given a baseline ImPACT Test during the first few weeks of school. ImPACT is a user-friendly, computer-based testing program specifically designed for the management of sports-related concussion.
