Health Information & Forms
Burroughs has a secure electronic health records system through which we maintain current and comprehensive information on your children. This system allows us to provide the best care for our students and provides a safe school environment for all. If your children have any additional health needs at school or if you have any questions about the medical forms, please contact Casie Tomlinson, school nurse.

Double check all information for accuracy and completion, and remember to include signatures where needed. We recommend making a copy for your own records in case you need to reference what you have submitted.
We depend on you to submit accurate and updated health information every year. All returning students must have a valid physical form on file. Missouri State High School Activities Association's guidelines allow physicals to be valid for two years from the date of the last exam, unless otherwise noted by the physician. If your child’s exam in our files is close to expiration, we will send an email to remind you to make an appointment with your physician. Please remember to submit the updated physical form and immunization record (if indicated) after your child’s physical.
All rising 8th grade students and rising 9th-12th grade students not participating in a fall sport: ALL FORMS ARE DUE NO LATER THAN AUGUST 16, 2021. Students must have all requested health information submitted in order to pick up their schedule.
All rising 9th-12th grade students participating in a fall sport: ALL FORMS ARE DUE BY AUGUST 9, 2021, the first possible day for fall sports practice. Students will not be able to participate in school sports practice until all requested health information has been submitted.
As part of our concussion management program, all new students will be given a baseline ImPACT Test during the first few weeks of school. ImPACT is a user-friendly, computer-based testing program specifically designed for the management of sports-related concussion. Visit ImPACT Test for more information.
