Health Information & Forms
Burroughs has a secure electronic health records system through which we maintain current and comprehensive information on your children. This system allows us to provide the best care for our students and provides a safe school environment for all. If your children have any additional health needs at school or if you have any questions about the medical forms, please contact Casie Tomlinson, school nurse.

(Remember, all 8th graders need a TB test, a TDAP booster and MCV and all 12th graders need a MCV.)

Please double check all information for accuracy and completion, and remember to include signatures where needed. We recommend making a copy for your own records in case you need to reference what you have submitted.
We depend on you to submit accurate and updated health information every year.
ALL PHYSICALS for the 2020-2021 school year must be completed and dated between August 2019 and August 2020.
DEADLINES
All forms for rising 9th-12th grade students participating in a fall sport need to be returned by August 10, 2020.
All forms for rising 8th-grade students and rising 9th-12th grade students not participating in a fall sport need to be returned by August 17, 2020.
Students will not be able to receive their schedules, purchase their books or participate in fall sports until all forms have been completed and submitted.
Concussion Information
As part of our concussion management program, all new students will be given a baseline ImPACT Test during the first few weeks of school. ImPACT is a user-friendly, computer-based testing program specifically designed for the management of sports-related concussion. Visit ImPACT Test for more information.
